29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: (a) On or about July 10, 2024, the employer did not ensure a rotating shaft end, located on the 5-Gallon Production Line conveyor system, which project greater than one-half the diameter. The shaft protruded approximately 3 1/8 inches from the conveyor system, with a diameter of apprimoximately1 1/8"1" in diameter", and was not guarded by a nonrotating cap or safety sleeve.
