On or about May 28, 2024, in the 6-feet deep trench, the employer exposed employees to a 13-feet fall hazard from the top of the egress ladder to the bottom of the storm drain, in that the access ladder did not extend at least 3 feet above the upper landing surface of the grade level for employees to access and egress from the storm drain connection.
