29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about April 24, 2024, employees who use locally manufactured extension cords at the T&T Quality Buildings facility located at 89384 558th Ave, Fordyce, Nebraska were exposed to electrical shock hazards because the cords had an electrical box installed not designed for handheld applications and one had missing knock outs. a. Temporary cord used to power a MIG Welder had a missing knockout with tape covering the hole. b. A Temporary cord used to power a MIG Welder had a junction box installed on it with a knockout.
