29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before further use On or about April 23, 2024, in the Engine Room #1 at Americold Logistics LLC Samuel facility, employees were exposed to fire, explosion and toxic gas release, when the employer failed to correct deficiencies of an undersized pressure relief valve on vessel V-2 (Low-Pressure Receiver).
