29 CFR 1910.184(f)(4)(ii): Welded end attachments were used without being proof tested by the manufacturer or equivalent entity at twice their rated capacity prior to initial use. In the framing department, employees were exposed to struck by hazards in that the wall assemblies were hoisted by a fabricated hook jig without a record of rated capacity. The employer did not obtain a proof test certification for the hook jig.
