29 CFR 1910.119(j)(5): Equipment deficiencies. The employer did not correct deficiencies in equipment that are outside acceptable limits before further use or in a safe and timely manner when necessary means are taken to assure safe operation. On or about April 9, 2024, in Alky Unit 22 at Phillips 66 Company located in Borger, Texas; the employer did not implement corrective actions for identified deficiencies. a) Deteriorated metal insulation jacket and damaged concrete support with exposed rebar for pressure vessels such as No. 3 B Tower Reboiler (22-41-R716) in Alky 22 Unit. b) Damaged/missing fireproofing on support structural steel and/or piping supports, to protect personnel and equipment from the effects of equipment or support failure during a fire in FCC 29 Unit.
