29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 4, 2024 the employer BMB Construction LLC at jobsite located at 41 Gone Fishing Ln. Lakeside/Marblehead OH 43440, did not ensure the side rails of one aluminum extension ladder was extended at least three feet above the landing surface. Two employees were exposed to falls from approximately 12 feet or greater from the top of the second (roof) level to the ground below while using a Werner brand extension ladder to access the second (roof) level to perform residential roofing activities on a single family home.
