29 CFR 1910.303(b)(1)(i): Electric equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees. Safety of equipment was not determined under suitability for installation and use in conformity with this subpart: a. On or about April 3, 2024, in the facility, the electrical extension cord utilized a 1900 electrical box as a junction point for a 250 volt plug to power welding equipment, thereby exposing employees to the hazards of electrical shock.
