29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about March 28, 2024, the employer did not ensure that the Tapping Machine Model 5NT-RM Serial-3632 was guarded to protect against crushing and amputation injuries. The ingoing nip points created by the metal cutting tool that performs the thread-cutting did not have any guarding to prevent employees from accessing the zone of danger. b) On or about May 22, 2024, the employer did not ensure that the point of operations was adequately guarded on the Threading Snow Machine Model 34 NTRM Serial-41766 to protect against crushing and amputation injuries. The hinged guard was frequently opened by employees while the machine was operating, making it ineffective to protect employees from the moving parts in the point of operations. c) On or about May 22, 2024, the employer did not ensure that the point of operations was adequately guarded on the Danly Air Press Model 84S92710017 to protect against crushing and amputation injuries. The ingoing nip points at the point of operation by the die that connects the metal parts did not have any guarding to prevent employees from accessing the zone of danger. d) On or about May 22, 2024, the employer did not ensure that the point of operations was adequately guarded on the Danly Air Press Model 84922701 to protect against crushing and amputation injuries. The ingoing nip points at the point of operation created by the die that connects the two metal parts did not have any guarding to prevent employees from accessing the zone of danger. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
