29 CFR 1910.178(l)(6): The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: a. 120 South 20th St., Irvington, NJ - Warehouse/Production areas The employer did not ensure that employees operating powered industrial equipment used in the movement of materials within the warehouse were provided an evaluation and certification prior to operating powered industrial equipment. Employees engaged in production and warehousing activities operated forklifts that included, but were not limited to, Caterpillar, TCM, and Toyota forklifts, and were not certified by the employer to operate the equipment. Violation occurred on or about March 27, 2024
