29 CFR 1910.146(c)(3): When the employer decided its employees would not enter permit spaces, the employer did not take effective measures to prevent its employees from entering the permit spaces: a. HK Cooperative, Inc. dba J. H. Routh Packing Company located in Sandusky Ohio: On or about April 11, 2024, the employer did not take effective measures to ensure employees were not entering permit required confined spaces. An employee was exposed to confined space hazards when entering equipment such as, but not limited to, the blood tank located in the blood room to perform cleaning activities.
