29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device prior to working on the machine or equipment: 38 Jackson Rd, Devens, MA: On and before October 24, 2023, employees did not always affix a personal lockout device prior to working on a machine locked out by another. On October 24, 2023 after a Metrology Technician received burn injuries from an uncontrolled pressurized steam release during removal of a pressure transmitter, a maintenance technician locked out the machine. Later the same day, the same injured Metrology Technician reinstalled the same transmitter into the same machine without placing his own locks or verifying the lack of energy.
