29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) On or about October 30, 2023, the Cushman electric utility tug, #1 charging power cord was missing the ground prong and not taken out of service, thereby exposing employees to electrical hazards. b) On or about October 30, 2023, the Cushman electric utility tug, #2 charging power cord was missing the ground prong and not taken out of service, thereby exposing employees to electrical hazards. c) On or about October 30, 2023, the Cushman electric utility tug, #3 charging power cord was missing the ground prong and not taken out of service, thereby exposing employees to electrical hazards. d) On or about October 30, 2023, the Cushman electric utility tug, #4 rear brake lights and front light were not functioning and not taken out of service, thereby exposing employees to electrical hazards. e) On or about October 30, 2023, the Cushman electric utility tug, #6 charging power cord was missing the ground prong and not taken out of service, thereby exposing employees to electrical hazards. f) On or about October 30, 2023, the Cushman electric utility tug, #8 charging power cord was missing the ground prong and not taken out of service, thereby exposing employees to electrical hazards. g) On or about October 30, 2023, the Cushman electric utility tug, #10 charging power cord was missing the ground prong and not taken out of service, thereby exposing employees to electrical hazards.
