29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access. Employees engaged in remodeling and restoration activities at the worksite located at 16281 Hwy 13, Jameson, Missouri were exposed to fall hazards in that the employer failed to ensure the employees accessing and egressing the upper level of a structure extended the portable ladder at least 3 feet above the upper landing surface.
