29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces were not protected from falling through holes more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) This violation occurred on or about August 1, 2023, when employee, who was working and walking close to the edge of holes more than 6 feet deep, was not protected from falling to the holes below.
