29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about July 19, 2023, the employer did not ensure that the point of operations on the #6 Accupress press brake was guarded. The press was foot petal activated and no guarding was in place to prevent the employee's hands from being between the dies when the press was cycled. The press brake had a laser guard installed on it, but it was muted and not functional. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d)
