29 CFR 1926.703(b)(6): Base plates, shore heads, extension devices, and adjustment screws were not in firm contact, and secured when necessary, with the foundation and the form: 188 Lafayette Street, New Haven, CT: On or about June 2, 2023, the employer did not ensure the base plates on the B1-B2 ramp were all placed in firm contact with the concrete ramp foundation where employees performed tasks such as (but not limited to) concrete work above three levels of shoring and re-shoring.
