29 CFR 1910.178(l)(4)(ii): (A) The employer did not provide refresher training to all employees engaged in the unsafe operation and use of forklifts: Instance a. On 25th May, 2023, at the US Postal Service Station DMDU Annex, Cata�o, an employee was observed operating a Toyota Electric Truck, entering the North platform and another employee driving another Toyota Electric Truck going in the opposite direction entering the Automated Parcel and Bundle Sorter (APBS) work area, using the same entrance. That entrance only has enough space for one vehicle at a time. This condition can cause caught by and struck by hazards in case powered industrial trucks coming from opposite sides, collide. Instance b. On 25th May, 2023, an employee was observed operating a Toyota Electric Truck, model 8HBE30, entering from the North platform into the Automated Parcel and Bundle Sorter (APBS) work area, using an entrance which was marked with a sign, 'WRONG WAY'. This condition can expose workers to struck by or caught between hazards during the transit of powered industrial trucks. Instance c. On 25th May, 2023, it was observed at the US Postal Service DMDU Annex in Cata�o, at the entrance that connects the North Platform and the Automated Parcel and Bundle Sorter, that, although there is a sign indicating the ONE-WAY direction when coming from the APBS area, and another sign indicating 'WRONG WAY' in the opposite direction coming from the North platform, there are arrows on the floor indicating the transit in both ways. This condition can cause confusion among the powered industrial truck operators about the flow of traffic and expose workers to struck by or caught between hazards during the transit of powered industrial trucks. NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c) and (d), respectively.
