29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the correct use and operation of personal fall arrest equipment. On or about May 19, 2023, and times prior thereto, employees were not trained on installing personal fall protection systems such as, but not limited to, anchors and adjusting rope grab lanyards to prevent contact with ground, exposing employees to fall hazards.
