29 CFR 1926.600(a)(6)(iv): A person was not designated to observe clearance of the equipment and give timely warning for all operations where it is difficult for the operator to maintain the desired clearance by visual means: On or about May 16, 2023 and times prior thereto, the employer did not ensure an observer was available to provide clearance for the forklift. This exposed employees to a struck-by hazards.
