29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles: (a)American Engineered Products, LLC at 605 4th Street Southeast, De Smet, SD 57231: On or about and at times prior to May 10, 2023, the employer did not ensure that the point of operation on the Amada press brake FBD 5020 machine was guarded to prevent employees from having any part of their body in the danger zone during operation, in that employees were not protected by safe distance, presence sensing devices, two-hand controls, pullback device, restraint device or physical guarding. This condition exposed employees to amputations hazards without effective machine guarding in place. (b)American Engineered Products, LLC at 605 4th Street Southeast, De Smet, SD 57231: On or about and at times prior to May 10, 2023, the employer did not ensure that the point of operation on the Amada RG-35S press brake machine was guarded to prevent employees from having any part of their body in the danger zone during operation, in that employees were not protected by safe distance, presence sensing devices, two-hand controls, pullback device, restraint device or physical guarding. This condition exposed employees to amputations hazards without effective machine guarding in place. This condition exposes to employees to amputations hazards without effective machine guarding in place. (e)American Engineered Products, LLC at 605 4th Street Southeast, De Smet, SD 57231: On or about and at times prior to May 10, 2023, the employer did not ensure that the point of operation on the Guifil PE 20-60 press brake machine was guarded to prevent employees from having any part of their body in the danger zone during operation, in that employees were not protected by safe distance, presence sensing devices, two-hand controls, pullback device, restraint device or physical guarding. This condition exposes to employees to amputations hazards without effective machine guarding in place. (f)American Engineered Products, LLC at 605 4th Street Southeast, De Smet, SD 57231: On or about and at times prior to May 10, 2023, the employer did not ensure that the point of operation on the Amada RG35 press brake machine was guarded to prevent employees from having any part of their body in the danger zone during operation, in that employees were not protected by safe distance, presence sensing devices, two-hand controls, pullback device, restraint device or physical guarding. This condition exposes to employees to amputations hazards without effective machine guarding in place. (g)American Engineered Products, LLC at 605 4th Street Southeast, De Smet, SD 57231: On or about and at times prior to May 10, 2023, the employer did not ensure employees were protected from amputation hazards by the use of appropriate guarding methods on the Mitsubishi Diamond press brake BH13530 in Kiosk Fabrication Area. The electronic safety device was deemed inoperable due to products blocking the laser optical system. This condition exposes employees to amputation hazards. (h)American Engineered Products, LLC at 605 4th Street Southeast, De Smet, SD 57231: On or about and at times prior to May 10, 2023, the employer did not ensure employees were protected from amputation hazards by the use of appropriate guarding methods on the Mitsubishi Diamond press brake BH13530 in Casino Fabrication/Metal Fabrication area. The electronic safety device was deemed inoperable due to the laser light not functioning correctly. This condition exposed employees to hazards such as amputation hazards.
