29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards: On or about April 26, 2023: Employees engaged in window replacement and upgrade on a residential home located at 2536 South 191st Circle, Omaha, Nebraska, were exposed to multiple fall hazards resulting from the employer's failure to ensure that employee(s) were trained to recognize any hazards associated with the use of a scaffold to perform work. Numerous scaffolding violations were observed during the inspection and a contributory factor was the absence of knowledge on the correct methods for erecting a scaffold. Instances include but are not limited to: i. Employees using a fiberglass extension ladder set up on top of the scaffolding to increase the working height. ii. Employees using scaffolding without fully planking each work level iii. Employees stepping up more than 2 feet to access the built in ladder of the scaffolding. iv. Employees working on the second level of the scaffolding without guardrails or another form of fall protection.
