29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions: (a) On or about March 29, 2023, and days prior, an employee was engaged in activities associated with the High-Bay florescent light fixture replacement upgrade project that resulted in exposure to energized 277 volt (AC) electrical current, did not utilize insulated class 00 or greater rated gloves, or electrical insulated tools. This exposed the employee to an electrical shock hazard.
