29 CFR 1910.119(f)(4): The employer did not implement safe work practices to provide for the control of hazards during operations such as control over entrance into a facility by maintenance, contractor, or other support personnel. These safe work practices were applied to employees and contractor employees. On or about March 22, 2023, in Cumene Plant at INEOS Phenol located in Pasadena, TX, workers were exposed to fire and explosion hazards, when the employer failed to implement an established safe work practice, " Rules and Requirements for Contractors - PHSP-104" for control over entrance into a facility, when the host employer instructed LPG/propylene tank truck drivers (contractors) to operate facility valves at truck unloading stations.
