29 CFR 1915.15(c): A competent person did not visually inspect and test each space certified as "Safe for Workers" or "Safe for Hot Work" as often as necessary to ensure that atmospheric conditions within that space were maintained within the conditions established by the certificate. a) Barge 141: On or about May 5, 2023 or at times prior thereto, employees entered a confined space to do cosmetic welding; however, the space was not tested by a certified marine chemist or USCG authorized person or visually inspected (no mechanical ventilation was provided) to determine atmosphere conditions while employees were actively inside the confined space.
