29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: On or about March 2, 2023, the employer did not ensure entry permits were completed prior to allowing employees to enter confined spaces, such as the #4029 Mississippi Tank Company manufactured tanker trailer. Employees entering the trailer were exposed to atmospheric hazards, such as oxygen deficiency and flammable gasses. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
