29 CFR 1910.184(e)(2)(ii): Makeshift links or fasteners formed from bolts or rods, or other such attachments, shall not be used. On or about March 1, 2023, and at times prior thereto, in the Fab Production Area, employees utilized a makeshift lifting device. Employees were exposed to struck-by hazards. 1. Employees utilized a shop made lifting device called a "Dog" connected to an alloy chain sling to lift and move material. 2. The device was not marked or was rated for capacity.
