29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: On or about January 27, 2023, the employer failed to ensure that when maintenance tasks were performed by a (Sanitation Department) crew of more than one employee group lockout procedures were utilized. This included, but was not limited to the application of personal lockout and tagout devices and coordination of the workforce when performing tasks on the Stewart Mono-Flex Grid Bun Cooler.
