The employer did not establish written procedures to maintain the on-going integrity of process equipment including the crystallizer. (b) Room 30 HEPES process - On or about 11/9/2022, the employer operates and maintains a covered process involving methanol, a class 1 flammable liquid having a flashpoint below 100F exceeding the TQ of 10,000 lbs. The employer did not establish written procedures to maintain the on-going integrity of process equipment including the crystallizer high-level shut-off. (c) Room 30 HEPES process - On or about 11/9/2022, the employer operates and maintains a covered process involving methanol, a class 1 flammable liquid having a flashpoint below 100F exceeding the TQ of 10,000 lbs. The employer did not establish written procedures to maintain the on-going integrity of process equipment including the crystallizer (AMETEK DR 3500 RADAR) level transmitter. (d) Room 30 HEPES process - On or about 11/9/2022, the employer operates and maintains a covered process involving methanol, a class 1 flammable liquid having a flashpoint below 100F exceeding the TQ of 10,000 lbs. The employer did not establish written procedures to maintain the on-going integrity of process equipment including the stainless steel pipe associated with the crystallizer. (e) Room 30 HEPES process - On or about 11/9/2022, the employer operates and maintains a covered process involving methanol, a class 1 flammable liquid having a flashpoint below 100F exceeding the TQ of 10,000 lbs. The employer did not establish written procedures to maintain the on-going integrity of process equipment including the BREDEL-50 hose pump. (f) Room 30 HEPES process- On or about 11/9/2022, the employer operates and maintains a covered process involving methanol, a class 1 flammable liquid having a flashpoint below 100F exceeding the TQ of 10,000 lbs. The employer did not establish written procedures to maintain the on-going integrity of process equipment, including the crystallizer vent line filter. (g) Room 30 HEPES process- On or about 11/9/2022, the employer operates and maintains a covered process involving methanol, a class 1 flammable liquid having a flashpoint below 100F exceeding the TQ of 10,000 lbs.
