The Yale forklift, Model GLC030BFNUAV084, Serial Number A809NO8872W. was being used with a counterweight that was not provided by the manufacturer. b. The Caterpillar forklift, Model C5000-LP, Serial Number AT9010666, was being used with front fork extensions that were not provided by the manufacturer.
