29 CFR 1910.146(d)(9): The employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and/or for preventing unauthorized entry. (a)On 8/15/2022 and at times earlier, in the Coke Hopper, contractor employees were exposed to falls of up to 13 feet to the bottom of the hopper, and hazardous energy at the point of operation for the vibrator because the employer did not develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, or for preventing unauthorized personnel from attempting a rescue.
