29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): Corner of Zeke and Galm Rd, San Antonio, TX 78254: On or about August 2, 2022, and times prior thereto, employees were exposed to a caught-in hazard while performing main gas line maintenance, inside an excavation with no cave-in protection in place at a depth of 6 feet below the ground.
