29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) Building #3 Fabrication shop: On or about 07/20/2022, the portable Lasko high velocity pivoting utility Fan was not visually inspected prior to use in that the male attachment plug was missing its ground prong. b) Building #3 Fabrication shop: On or about 07/20/2022, the metal portable industrial pedestal fan was not visually inspected prior to use in that the outer jacket of the flexible cord was pulled away the male attachment plug thus caused tension on the internal connections. No abatement certification or documentation required.
