29 CFR 1910.119(h)(2)(v): The employer did not periodically evaluate the performance of contract employers in fulfilling their obligations as specified in paragraph (h)(3) of this section. On or about April 23, 2022, in the ROSE unit at Valero Meraux refinery located in Meraux, LA, workers were exposed to fire and explosion hazard, when the employer did not evaluate Elite Turnaround Specialists (ETS) to assure that the contract employees have knowledge, training and training records/certification for line break on process piping.
