29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of a powered industrial truck were performed by the employer without the manufacturer's prior written approval: On April 4, 2022, and times prior to, Craftsmen Industries, Inc. located at 3101 Elm point Industrial Drive, Saint Charles, MO did not ensure powered industrial truck(s) using fork attachments had not received approval from the manufacturer of the forklift. TCM, Model # FCG25F9, Serial # A12W01106, was equipped with a set of fork extensions measuring 72 inches long. Forks without extensions measure 41 inches long.
