Text For Citation: 01 Item/Group: 005 Hazard:
29 CFR 1910.147(f)(1):In situations in which lockout or tagout devices had to be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the sequence of actions described in 1910.147(f)(1)(i)-(v) was not followed. (a) On or about 3/9/2021, and at times prior, at 110 Plugs Drive, Decatur, AL 35601, on Line 49, employees working to thread/clear jam of sealing substrate on VisionPak employees were exposed to unguarded moving machine parts which could cause contusions, fractures, or disfigurement including amputation because when VisionPak components needed to move to thread the film, he sequence of actions described in 1910.147(f)(1)(i)-(v) was not followed.
