29 CFR 1910.119(d)(3)(i)(D): Information pertaining to the equipment in the process did not include relief system design and design basis: a) Polymerization Plant 2: The employer did not ensure that information pertaining to the equipment in the process including the relief system design and design basis was maintained for the reactors. Reactors R1-4 relief outlet system design basis was not updated to ensure the rupture disc sizing and temperature were appropriate. Failure to adequately design and size relief systems can lead to uncontrolled releases during a runaway reaction resulting in employee exposure to fire and explosion hazards, on or about February 2, 2022. b) Polymerization Plant 2: The employer did not ensure the information pertaining to the equipment in the process including the relief system design and design basis was maintained for the de-volitizer vessels. De-volitizers (D1 and D2) relief outlet design basis and sizing were not included in the relief scenarios. Failure to adequately design and size relief systems can lead to uncontrolled releases during a runaway reaction resulting in employee exposure to fire and explosion hazards, on or about February 2, 2022.
