Text For Citation: 01 Item/Group: 005 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to struck-by/fire/explosion hazards associated with the improper control of potential ignition sources in the fill station area of the facility while working with and around liquified petroleum (propane) gases: a) Lowbucs Propane, Inc., Fill Station area, On or about January 31st, 2022 and times prior thereto: Various liquified petroleum gas (Propane) cylinders including, but not limited to, 33 pound cylinders and 20 pound cylinders were being filled and stored within the fill station area of the facility. Employees were exposed to explosion hazards from general sources of ignition including, but not limited to: Manual Pallet Truck/Jack (Model: H1365 ; Serial#565310) during the maneuvering of pallets of liquified petroleum cylinders within the fill station area. b) Lowbucs Propane, Inc., Fill Station area, On or about January 31st, 2022 and times prior thereto: Various liquified petroleum gas (Propane) cylinders including, but not limited to, 33 pound cylinders and 20 pound cylinders were being filled and stored within the fill station area of the facility. Employees were exposed to explosion hazards from general sources of ignition including, but not limited to: Potential spark producing tools during work operations with liquefied petroleum cylinders within the fill station area. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
