29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by pinch points, in-going nip points, rotating parts, flying chips and sparks: A. Kummer Cell, Machine #1213: On, or about January 10, 2022, employee's standing on a work platform loading/unloading parts from an automatic feeding conveyor were exposed to rotating parts on the back of the chuck/spindle. B. Production Floor: On, or about, January 10, 2022, a guard was not provided on the chuck of a Shop Fox drill press that was used to ream parts. C. Grinding Department: On, or about, January 21, 2022, the pinch point created by the robot mounted jaw used to load parts into Machine #0641, a VanNorman grinder, was not safeguarded. D. Grinding Department: On, or about, January 21, 2022, the pinch point created by the robot mounted jaw used to load parts into Machine #0624, a Micron grinder, was not safeguarded. E. BCNC: On, or about, January 21, 2022, the following Tsugami B0306-III screw machines were equipped with Minuteman 320 SE stock feeders that had exposed moving parts where the machines were connected: #2510, #2509 and #2508. A pinch point was formed between the stock feeders and the side of the screw machine. F. Compensator Cell: On, or about, January 21, 2022, Machine #4115, a Bridgeport CNC milling machine, was equipped with a conveyor to feed and remove parts from the point of operation. In order to accommodate the conveyor system, a panel was positioned between the two doors that had openings for the feed and discharge. In the event of tool breakage, shattered parts could be thrown from the machine enclosure. G. Compensator Cell: On, or about, January 21, 2022, Machine #4116, a Bridgeport CNC milling machine, was equipped with a conveyor to feed and remove parts from the point of operation. In order to accommodate the conveyor system, a panel was positioned between the two doors that had openings for the feed and discharge. In the event of tool breakage, shattered parts could be thrown from the machine enclosure. H. IMAS Area: On, or about, January 31, 2022, Machine #1267, an Accu-Cut vertical boring machine, was operated with a two hand trip to start the machine cycle and was released after the boring tool made contact with the part. The rotating tool and chuck were not protected against accidental contact.
