29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: On or about December 22, 2021, in the the Hydro Desulfurization Unit (HDU-1)at ExxonMobil Refinery in Baytown, Texas, the employer failed to perform an adequate assessment to determine and address the workplace and did not select and have employees and contractors use personal protective equipment that would protect them in the event of a fire while working on/near a Naphtha flange leak.
