29 CFR 1926.201(a): Signaling by flaggers and the use of flaggers, including warning garments worn by flaggers did not conform to Part VI of the Manual on Uniform Traffic Control Devices, (2009 Edition, Revision 1 & 2): a) W 3rd Street & N Broom Street (Between N Broom St & N Clayton St: On or about December 22, 2021, employer failed to ensure flaggers followed the standard operating procedures of Section 6E.07 of the 2009 Edition, Revision 1 &2 of the Manual on Uniform Traffic Control Devices (MUTCD). Flaggers were not facing oncoming traffic with the Slow/Stop facing toward road users. Employees were exposed to multiple hazards onsite including but not limited to struck by/ caught in hazards when they performed flagging activities during lane closures for concrete improvements.
