Text For Citation: 01 Item/Group: 005 Hazard:
29 CFR 1910.157(d)(1): Portable fire extinguishers were not selected based on the classes of anticipated workplace fires and on the size and degree of hazard that would affect their use: a) Boulder Scientific Company, LLC., 598 Third Street, Mead, CO 80542: On or about October 22, 2021, the fire extinguishers used to try to extinguish the fire were Class ABC extinguishers instead of Class D, which were not suitable for fires that are associated with pyrophoric chemicals. The chemicals used in this room, butyl lithium and lithium hydride were water reactive. While in the process of trying to contain an incipient stage fire employees were unable to put out the fire out until soda ash/ dry chemicals were used. The employee, as well as, other in the immediate area were not protected from fire hazards.
