29 CFR 1910.28(b)(3)(ii): 29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. a. 156 West Harrisburg Avenue Elizabethtown, Pennsylvania - On or about October 19, 2021, in the packing room, there was a 17 x 24 inch floor hole at the top of the mezzanine stairs, exposing employees to a 7 foot 4 inch fall hazard. b. 156 West Harrisburg Avenue Elizabethtown, Pennsylvania - On or about October 19, 2021, in the packing room, there was a 6 x 46 inch floor hole between the Hagen 1 and Hagen 2 platform, exposing employees to a 7 foot 4 inch fall hazard. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
