29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: a) Lot 35 at the Showfield development in Lewes, DE 19958: On or about September 10, 2021, the employer failed to ensure employees used ladders that extended at least 3 feet above the upper landing to access the roof, exposing employees to fall hazards approximately 13 feet above grade.
