29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a.) Warehouse - On or about August 27, 2021, the employer did not provide effective guarding to protect employees working at or near the Logemann Alligator Shear, Serial No. 15145. An employee's right wrist was severely cut while operating the alligator shear, which resulted in a right hand amputation. b.) Warehouse - On or about September 8, 2021, the employer did not provide effective guarding to protect employees working at or near the Logemann Alligator Shear (unknown serial no.), thereby exposing employees to amputation hazards.
