29 CFR 1910.119(e)(3)(vi): The process hazard analysis did not address human factors. a) Chlorine process: The PHA did not address human factors when changing one ton chlorine containers. The PHA did not address using improper torqueing or using the wrong type of wrench when connecting and disconnecting the containers, and tightening the packing nut, on or about 8/10/21. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
