29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) Hole # 000-008 Between mile marker 6 and 10, HWY 64, Lone Mountain Trail, Big Sky, MT: On or about July 14, 2021, and at times prior, employees were exposed to cave in hazards while hand digging concrete piers foundations. The employer failed to have a competent person conduct an inspection of the excavation prior to entry. b) Hole # 000-011 Between mile marker 6 and 10, HWY 64, Lone Mountain Trail, Big Sky, MT: On or about July 23, 2021, and at times prior, employees were exposed to cave in hazards while hand digging concrete piers foundations. The employer failed to have a competent person conduct an inspection of the excavation prior to entry. c) Hole # 000-012 Between mile marker 6 and 10, HWY 64, Lone Mountain Trail, Big Sky, MT: On or about July 22, 2021, and at times prior, employees were exposed to cave in hazards while hand digging concrete piers foundations. The employer failed to have a competent person conduct an inspection of the excavation prior to entry. d) Hole # 000-013 Between mile marker 6 and 10, HWY 64, Lone Mountain Trail, Big Sky, MT: On or about July 23, 2021, and at times prior, employees were exposed to cave in hazards while hand digging concrete piers foundations.
