29 CFR 1910.119(h)(2)(ii):The employer did not inform contract employers of the known potential fire, explosion, or toxic release hazards related to the contractor's work and the process: On or about February 27, 2021 in the process area that includes Unit 6 and Unit 9 of the Lion Oil Refinery in El Dorado, AR, workers were exposed to fire, explosion and toxic release hazards when the employer did not inform contract employers that temporary piping in the roadway between the units connected process units and contained flammable hydrocarbons. (a) Contract employer SWAT had workers interacting with the piping and was not informed of the process hazards. (b) Contract employer Turn 2 had workers in the area of the piping and was not informed of the process hazards.
