29 CFR 1910.146(k)(2)(ii): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not train affected employees to perform assigned rescue duties. The employer must ensure that such employees successfully complete the training required to establish proficiency as an authorized entrant, as provided by paragraphs (g) and (h) of this section: (a) On or about 10/27/2020 - Chamber 40, employees conducting cold climate testing entering permit spaces such as, but not limited to, the foam-constructed enclosure inside Chamber 40 had not been trained exposing employees to confined space hazards, including, but not limited to, low visibility, limited access, and oxygen deficiency.
