29 CFR 1910.147(f)(4): During shift or personnel changes, specific procedures were not utilized to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout devices between off-going and oncoming employees, to minimize exposure to hazards from the unexpected enerization or start-up of the machine or equipment, or release of stored energy. a) At the facility, on or about September 3, 2020, the employer allowed an employee to remove his locks and tags at the end of his shift without ensuring the machine was relocked and retagged by another employee, thereby exposing employees to crushing, struck-by and caught-in hazards.
